The following are required by the State of NJ for each student-athlete.
- As of the start of the 2014 school year, the Camden Diocese being mandated by the State of NJ, and in accordance with P.L. 2013, c71 the Scholastic Student-Athlete Safety Act, will require all students in grades 6-12 to have on file with each school’s nurse, the Preparticipation Physical Evaluation form. This form needs to on file with each school before the student participates in any interscholastic or intramural sport. New roster forms to be submitted to sport coordinators, now have to be certified if students have this form on file by using the “PPE Form” checkbox. - Schools must also distribute the Sudden Cardiac Death in Young Athletes pamphlet to each student-athlete, pursuant to N.J.S.A. 18A:40-41(d) . All student-athletes and their parents (or guardians) shall certify to each school that they have received and reviewed the pamphlet. - The Concussion Information Sheet must also be filled out and signed by all student-athletes and their parents (or guardians), according to the CDC and the 3 rd International Conference on Concussion in Sports.
